1. Death of a child or youth
2. Child is AWOL and/or Returning from AWOL (Included in Leaves)
   1. Please have police report details (report #, date and time initiated/cancelled) available to complete report
3. Any serious injury or illness involving initial non-routine medical treatment
   1. *Emergency medical treatment is if a child was seen at a medical facility the day of the incident
   2. Include leave type hospitalization when applicable
4. Suspension or Expulsion from school
5. Any alleged delinquent/criminal activity of child
   1. Include leave type detention when applicable
6. Child is a victim of any alleged delinquent or criminal activity
7. Any involvement with law enforcement
8. Suicide or self-mutilation attempt
9. Use of physical restraint or isolation
10. Any incident of alleged abuse or neglect
11. Any other unusual incidents as defined in the provider agency's policies or by the provider agency
12. Any other leave or return from leave
