Incident Report/Leaves that are required to be reported through this process
- Death of a child or youth

Child is AWOL and/or Returning from AWOL (Included in Leaves)
- Please have police report details (report #, date and time initiated/cancelled) available to complete report
- Any serious injury or illness involving initial non-routine medical treatment
- *Emergency medical treatment is if a child was seen at a medical facility the day of the incident
- Include leave type hospitalization when applicable
- Suspension or Expulsion from school

Any alleged delinquent/criminal activity of child
- Include leave type detention when applicable
- Child is a victim of any alleged delinquent or criminal activity
- Any involvement with law enforcement
- Suicide or self-mutilation attempt
- Use of physical restraint or isolation
- Any incident of alleged abuse or neglect
- Any other unusual incidents as defined in the provider agency's policies or by the provider agency
